High Pathogenic Avian Influenza (HPAI)
State: All
Date Effective: May 14, 2015

All poultry moved into New York require a Certificate of Veterinary Inspection or a USDA VS Form 9-3 and must be:
1) From a source flock in which 30 birds were tested negative for avian influenza within ten days prior to entry into New York State; Or
2) From a source flock that has been certified by the State of Origin as an Avian Influenza Monitored Flock; Or
3) From an NPIP U.S. H5/H7 Avian Influenza Clean or NPIP U.S. Avian Influenza Clean Flock

All hatching eggs and day-old chicks imported into New York must be from an NPIP U.S. H5/H7 Avian Influenza Clean flock or NPIP U.S. Avian Influenza Clean Flock.
